You may also use our website or app to requests appointments. It is also best that at the end of your appointment, you schedule a follow-up prior to leaving.

Yes, you can see whoever you would like. If there is a provider you would rather see, please make a note when requesting an appointment or advise one of our clerks.

At this time, we are not accepting in-office sick visits. We do however offer virtual visits via telehealth for same-day appointments. If we feel we need to see you in person due to a particular concern or for a lab/diagnostics, we will work to do so.

Yes. On our website, go to the Patient Portal heading, click New Patients, and select the downloadable form of the New Patient Paperwork.

We are working to set up a way to be able to pay bills online. At this time, you may use our Healow app or pay in person, call, or through mail.
